You must provide us with all documentation specified in this checklist.  If you do not, it may delay the issuing of the certificate.

All identification documents lodged with your application must be original documents or copies certified by a Justice of the Peace (JP) or Authorised Officer as being a true and correct copy of the original document.

NEW ISSUE

  Application Form & Payment of Fee

  Character Declaration, Passport Photo and 100 Point Identification form

  Certificate of Medical and Eyesight Fitness form

  Qualifying Sea Service / Practical Experience

• Record of Service Book and supporting Testimonial; or Statutory Declaration.
• ROPES Book (optional)

  Elements of Shipboard Safety (or OH&S at Sea) Certificate

 Current Senior First Aid Certificate

  Marine Radio Operator’s Certificate of Proficiency

  Trade Certificate (if relevant)

  Application Form & Payment of Fee

  Proof of Identity (photo driver’s licence acceptable in lieu of 100 points of identification)

  Certificate of Medical Fitness for Revalidation of a Marine Certificate of Competency

  Qualifying Sea Service / Practical Experience

• Record of Service Book and supporting Testimonial; or Statutory Declaration

  Current Senior First Aid Certificate
